After a significant sprain, two things change. The ligament heals at a slightly greater length, and the proprioceptive feedback from the joint — your unconscious sense of where your foot is in space — is disrupted.

Both respond to targeted rehabilitation: peroneal strengthening, balance and reactive work, and progressive loading through positions that currently feel unsafe.
What to do in the first week
Early management shapes the outcome more than anything that follows. Protected weight-bearing as tolerated, early gentle range of motion, and avoiding prolonged immobilisation all produce better results than complete rest.
If you cannot weight-bear four steps, or there is bony tenderness over the malleoli, that warrants imaging before rehabilitation begins.
