The first metatarsophalangeal joint — the base of the big toe — is the most frequently affected. Stiffness there, known as hallux rigidus, limits the toe extension needed for a normal push-off, which is why people often notice it first as a change in how they walk rather than as joint pain. The midfoot and ankle are also common sites, particularly after previous injury. Management centres on reducing the load through the affected joint while maintaining overall mobility: footwear with appropriate rocker or stiffness, orthotic modification, targeted strengthening around the joint, and activity modification that preserves rather than removes movement.

Rheumatoid and psoriatic arthritis affect the foot early and often, and the priorities differ from osteoarthritis — protecting joint integrity, managing pressure areas over deformity, and monitoring skin in patients on immunosuppressive therapy.
