Plantar heel pain that persists past three months usually falls into one of three groups. The first is a wrong diagnosis: fat pad atrophy, a calcaneal stress reaction, or nerve entrapment presenting as plantar fasciitis. The second is an under-dosed loading programme — the tissue needs a progressive strengthening stimulus, and gentle stretching alone does not provide one. The third is an unaddressed driver, most often limited ankle dorsiflexion or a sharp change in weekly load. A thorough reassessment sorts which applies, and the plan changes accordingly.
What comes after conservative care
Where a well-executed conservative programme genuinely plateaus, there are further options — shockwave therapy, injection, and in a small minority surgical referral. Those come after the basics have been done properly, not instead of them. Most people who believe conservative care has failed have not yet had it delivered at an adequate dose.
