Medial tibial stress syndrome, commonly called shin splints, is diffuse pain along the inner border of the shin that typically eases as you warm up and returns afterwards. It is a load problem and responds to load management, calf and foot strengthening, and gait or footwear adjustment. A tibial stress fracture behaves differently. The pain is focal rather than diffuse, worsens through activity rather than easing, and may be present at rest or at night. That distinction matters enormously, because continuing to run on a stress fracture risks progression to a complete fracture. If your shin pain is localised to a point you can cover with a fingertip, stop running and get it assessed.
Returning to running safely
Return is graded on symptom response rather than the calendar. We set a weekly load progression, define what an acceptable symptom response looks like, and adjust from there. We also address why it happened — a jump in volume, a change of surface, worn-out footwear, or insufficient calf capacity for the distances being run.
