A Morton’s neuroma is a thickening of nerve tissue between the metatarsal heads, most often between the third and fourth toes. The classic description is burning or electric pain radiating into the toes, sometimes with numbness, usually worse in tight or high-heeled shoes. But metatarsalgia, a plantar plate injury, a stress reaction and bursitis all produce forefoot pain, and the treatment for each differs. A plantar plate injury in particular is frequently mistaken for a neuroma and made worse by the standard neuroma advice. Diagnosis comes from a careful clinical examination and, where the picture is unclear, imaging.
What actually helps
For most forefoot conditions the first line is offloading — changing where pressure sits during push-off, through footwear, padding or an orthotic modification. This is unglamorous and highly effective. Where conservative care plateaus, we will discuss injection or surgical referral pathways honestly, including what the evidence says about each.
